New SSS Contribution Payment Form by 01 SEPTEMBER 2014

In its effort to streamline procedures and to make it more convenient for its members, the Social Security System (SSS) has consolidated its contribution payment forms for individual payors.

Starting 01 September 2014, voluntary, self-employed, OFWs and non-working spouses will be required to use the new consolidated contribution payment form.

Employers will continue to use the SSS Form R5 in the payment of their employees’ contributions until the current stock of the form is consumed. Employers are required to start using the new contribution payment form on 01 November 2015.
